Homelessness Coordinated Access System

The City works alongside community organizations in a Coordinated Access approach that supports individuals experiencing or at risk of homelessness by assessing each unique situation and connecting them to financial, social services, health, and support systems. This coordinated network is called the Brantford-Brant Homelessness System of Care and includes the following organizations:

Coordinated Access aims to bring consistency to processes that deliver services for those experiencing homelessness or at risk of experiencing homelessness. Core components of Coordinated Access include:

  • A Housing-First approach, real-time data about the supply of and demand for housing and homelessness resources
  • A streamlined service delivery approach with clear access points to service
  • Standardized workflow for triage and assessment; prioritization; and vacancy matching and referral. (Reaching Home Definition)

A By-Name List (BNL) is a real-time list of all known people experiencing homelessness in Brantford-Brant.  It includes a robust set of data points that support coordinated access and prioritization at a household level and an understanding of homeless inflow and outflow at a system level.  This real-time actionable data supports system performance evaluation and advocacy for the policies and resources necessary to end homelessness.

The Homeless Individuals and Families Information System (HIFIS) is a comprehensive data collection and case management system designed to better understand what is happening in our community and to work collaboratively.

The Brantford-Brant Coordinated Access System tailors services to meet the client’s needs and level of readiness. Brantford-Brant has a centralized access system which includes the Housing Resource Centre, and after-hours Emergency Intake services, which are contracted services provided through SOAR Community Services. Clients can access services in person or by phone and can self-refer or connect through support from a community agency, or a City of Brantford outreach worker (Community Initiatives Coordinators).

Entry Point Management

Staff complete “Entry Point Management” activities with the individual, including the completion of consents, creating or updating individual information on HIFIS, and the provision of diversion services. The goal of entry point management and diversion is to learn about an individual’s specific housing situation, and to determine if safe alternatives to housing are available.

Accessing Safe Shelter

When possible, the individual/family is diverted away from emergency shelter accommodations and is provided with resources or supports that may help them to access a suitable housing option. If an individual/family is unable to be diverted, the client is admitted to an available emergency shelter bed.  

Case Management

Once admitted to shelter, staff support the client with case management activities. Case management includes the completion of an assessment and conversations to determine appropriate community supports.  The Brantford-Brant Coordinated Access System works collaboratively and with many additional agencies to provide the supports clients require. Information gathered during assessments and case management activities determine risk levels, housing needs, and support services required for housing stability on an ongoing basis. A primary goal during case management is completing housing applications.

Suitable Housing

If a suitable housing opportunity is identified for an individual/family, the individual is referred to the permanent housing option, and supports are offered to ensure the transition is successful.

Coordinated Access

The City of Brantford’s Housing Stability team, in consultation with the Coordinated Access System agencies, aim to develop clear and consistent policies and protocols for the delivery of the emergency shelter and supporting services. This sets clear expectations for providers and clients.

Reaching Home Community Advisory Board (CAB)

The Reaching Home Community Advisory Board (CAB) includes community representatives, and is led by the City of Brantford. CAB monitors the allocation of funding for homelessness-related initiatives, identifies service needs and gaps, and ensures Reaching Home funds support activities that align with local needs.  Learn more at the Reaching Home Community Advisory Board webpage.
